Family Alliance responds to seniors who have experienced situational, medical, and/or psychological problems which interfere with thier ability to accomplish the tasks of daily living with preventive/interactive Senior Day Health Services, Counseling, Dementia Support Services, Case Management, and Care Management Programs. These programs provide maintenence and/or prevention of the excessive deterioration of "at risk" seniors needing services. Trained staff respond to the needs of the senior to allow the family to receive needed respite so thay can continue to provide the physical and emotional support to their family member.
Adult Day Health Services
This program is designed to provide social interaction, intellectual and social stimulation and professional response to the comprehensive care needs of the medically and/or mentally frail senior, the senior suffering from dementia or the senior who is isolated and cannot manage on his own.
Consultation & Training
Family Alliance staff provides seminars and workshops to local social service agencies, state and local adult day program and nursing associations, and state regional and national senior organizations (Illinois Department on Aging, American Society on Aging, Mid-America Council on Aging, National Council on Aging, etc.).
Care Management
This program includes: 1) Assessment of senior and evaluation of need to identify health, personal, cognitive, financial and social support needs of the senior. 2) In-office consultation with key family members to think through available options and resources to meet the older person's needs. 3) Implementing the plan of care, determining the extent to which services are needed to maintain the independence of the senior. Services needed may include: in-home care, counseling, day care, respite, etc.
Case Management
Family Alliance offers a Case Manager to provide client-centered, individual assistance to advocate for and obtain necessary social and health care services.
Alzheimer's Services and Support Group
We offer supportive services, education, and programs to provide respite to the caregivers. We meet the individual with at their present level and help them in their response to the illness.
Caregivers Support Group
The goal of the Caregivers Support Group is to provide basic information on resources needed by caregivers and a supportive atmosphere where caregivers can gather and share their concerns. The support group offers educational programs so that members can learn more about various aspects of aging and how this affects their caregiving situation.
Dementia Support Services
Caregivers responding to the needs of a family member with dementia can receive assessment, education, support services and follow-up in the comfort of their own home. The goal of the program is to enhance the quality of life for the family member with dementia and broaden the base of support for the caregiver.
Senior Counseling/Therapy
The objectives of this counseling program include: Offering each senior-in-need a supportive, therapeutic atmosphere where they may express their frustrations, share their anxieties, and unashamedly grieve their losses; Providing individual and group support to the seniors and their families so that an understanding of the aging process and its accompanying physical, mental and emotional complications may be gained. Mental Health Services include: 1) Geriatric Psychiatric Consultation and evaluation, 2) Psychiatric Assessment, Diagnosis and Follow-up, 3) On-Site and Out-Reach Counseling/Therapy, 4) Individual and Group Counseling/Therapy, 5) Information, Advocacy and Referral, 6) Long-Term Care Monitoring.
